1. COVID-19 Vaccine Booster Shots

Booster shots are available for all people who have had the 2nd dose of their primary COVID-19 vaccination series at least 6 months ago, and it is free of charge.

You will receive an envelope from your city hall.

The envelope includes the following items.
① Vaccination Voucher [SESSHUKEN]
② Notification of Booster Shot for COVID-19 Vaccine (3rd dose)
③ Screening Questionnaire for COVID-19 vaccine

Vaccination Certificate of 1st and 2nd dose is necessary to take the booster shot. If you wish to make an appointment for the booster shot, please prepare the certificate.

2. Cash Assistance for Low Income Households

If your address was registered at the municipal office at the time of December 10, 2021 and you are a low income household, you are eligible to receive this cash assistance (100,000 yen) from the municipal office where you resided as of December 10, 2021.

Les injections de rappel sont disponibles pour tous ceux qui ont reçu la 2e dose de leur série de vaccination primaire COVID-19 il y a au moins 6 mois, et c’est gratuit.

Vous recevrez une enveloppe de votre mairie.

L’enveloppe comprend les éléments suivants.

③ Questionnaire de dépistage du vaccin COVID-19

Le certificat de vaccination de la 1ère et de la 2ème dose est nécessaire pour faire le rappel. Si vous souhaitez prendre rendez-vous pour la piqûre de rappel, merci de préparer l’attestation.

1. 新型（しんがた）コロナウイルス・3回目（かいめ）のワクチン：

新型（しんがた）コロナウイルスのワクチンは、誰（だれ）でも無料（むりょう）で受（う）けられます。
3回目（かいめ）のワクチンは、1回目（かいめ）と2回目（かいめ）の接種（せっしゅ）を6ヶ月（ろっかげつ）以上（いじょう）前（まえ）に受（う）けたという証明書（しょうめいしょ）を持（も）っている人が受（う）けられます。

市役所（しやくしょ）からの手紙（てがみ）の中（なか）には、新型（しんがた）コロナウイルス・3回目（かいめ）のワクチンについての、

家（いえ）に届（とど）く手紙（てがみ）は捨（す）てないでください。JARへ連絡（れんらく）してください。

2. 給付金（きゅうふきん）のおしらせ：

2021年（ねん）12月（がつ）10日（にち）に住民票（じゅうみんひょう）があり、家族（かぞく）全員（ぜんいん）の給料（きゅうりょう）が低（ひく）いひとは、住民登録（じゅうみんとうろく）がある市役所（しやくしょ）から、お金（かね）（10万円（まんえん））がもらえます。わからない人（ひと）は、JAR 0120-477-472 (フリーダイアル）/ 03-5379-6003 に電話（でんわ）してください。
